I acknowledge that basketball presents inherent risks and hazards, and I urge my child to consider these risks before participating. By signing below, I approve of my child's participation in the Jared Armstrong Basketball Camps, INC basketball program and consent to emergency medical treatment for my child on my behalf. I certify that, to the best of my knowledge, there are no physical or other conditions that will interfere with my child's participation.
